forms for new clients
New client forms will be sent to families via email or post once an appointment time is confirmed.
Forms must be completed and returned before your appointment date, giving our practitioners sufficient time to review your concerns and background history prior to meeting you.
All of our new clients pass through an intake process. This ensures that we are able to fully meet your needs and assign the best fit for therapy.

Information about our service provision
parking
2-hr parking is available out the front of our office on Grey Street. Parking is also available at the rear via the laneway on Grey Street beside the Premier Function Centre (no rear access to think.grow.connect - please enter via door on Grey Street).
Parking is also available at Stockland Shopping Centre, which is roughly 200m from our office.
